There are some insecticides that are of relatively low impact upon humans and seem to maintain efficacy against insects. I don't think it really matters where the flight originates. An infected mosquito leaving an airplane in the US or Canada has a pretty low chance of surving to transmit malaria. It could happen, but even if it did, a single case will unlikley lead to an epidemic as malaria gets diagnosed and treated fairly promptly. There have been about 20 cases of locally aquired malaria in the US since 1986, all have been quickly identified and dealt with. The public health infrastructure is pretty good. It is probably much better to respond where necessary, than to needlessly treat every aircraft with insecticides. While humans are a reservoir for malaria and other insect vectored diseases, we may discover that they can be transmitted by other than the usual vector and hosted by unsuspected species. Malaria existed in the U.S. up into the 1930's in areas as far north as lat 37. Recent news in FL indicates that some of our local mosquito species are now transmitting malaria just as yours in Canada are doing. The Florida cases were considered to be the result of "extended transmission from a single infected person". (http://www.cdc.gov/mmwr/PDF/wk/mm5238.pdf ) That means to me that it was local mosquitoes that aquired malaria and tranmsitted it. Spraying airplanes with insecticides would have done nothing to prevent an infected person from bringing the parasite in and infecting local mosquitoes. There is no mosquito transmission of malaria in Canada at present. All cases are imported in humans. Malarai has been transmitted in Canada in the past. An outbreak occurred in Ottawa in the 1800's. It was caused by an influx of infected British military personnel who came from India. You are arguing based upon known factors and to some degree upon those factors being stable. I'm arguing based upon probabilities that new vectors and hosts will emerge as travel and trade allow. I hope you are correct. There is a lot known about malaria. I doubt new hosts will emerge, and the vector species are well known, including the ones in the US. Tim Lysyk |

With regard to Malaria, this may be true. I'd have to spend some time pulling data to dispute your numbers re locally acquired cases and would likely find much the same as you cite. In the years since I entered clinical practice, I've seen four or five cases. At least one of these involved a patient with no history of travel outside the CONUS. Another could possibly have been a relapse from infection at an early age. But the public health response is also hampered by the lack of willingness of some populations to provide required information. I've worked with state epidemiologists in tracking one large Hep A outbreak and was amazed at how uncooperative part of the infected population was. I can't begin to imagine trying to track outbreaks in a locale like Miami where immigration status is also a factor in cooperation. There is a lot known about malaria. I doubt new hosts will emerge, and the vector species are well known, including the ones in the US. No argument here.
