It gets in via a leaky seal -- usually the prop shaft seal, although the
shift shaft seal is another common culprit on some motors.

If you live in a sub-freezing winter climate, it will crack the lower
unit housing when it freezes over the winter. Other than that, it foams
the oil and causes it to become a much less effective lubricant. So,
yes, you need to get it fixed.
