Default Worm Help.

Can someone tell me the proper way to put a worm on a hook? It seems that no
matter how I put a nightcrawler on a hook, the fish seem to bite or suck it
right off without a problem.

Is there a special rig or something that will make my worm last longer? I'm
pretty fed up with casting a worm and a minute later, reeling it in only to
find 1/4" of worm still hanging on.

Any help would be greatly appreciated.
